In process plants, the record is part of the control.

Pumps, reactors, exchangers and relief systems carry hazardous material. Permits, gas tests, near-miss reports and procedures are not admin. They are how the plant stays safe.

Start a pilot

A pump seal flush line blocks on a night shift.

An illustrative night on this kind of floor, and what Plant Cortex does at each point.

  1. The signal

A transfer pump starts running hot and the seal pot level is dropping. Product is behind that seal.

  2. The question

The operator asks what to check. The answer cites the pump and seal manuals, calls out the flush plan, and says plainly where the manuals are silent instead of improvising.

  3. The work

A confined-space style permit carries the gas-test fields with pass and fail against thresholds. A near-miss is filed from the same screen and routed to the right department.

  4. What stays

The near-miss joins others in a cluster view. Three reports about the same flush arrangement become one thing to fix, not three forgotten forms.

Where this floor gets hurt.

Near-misses that never get reported

A form nobody fills in tells you nothing. Reports can be anonymous, are routed to a department, and are acknowledged by a person.

Gas tests and atmosphere

Oxygen, LEL, H2S and CO readings with pass or fail against stated limits. Real readings are always entered by a person, never invented.

Procedures that drift from the manual

A suggested change to a checklist is re-checked against the equipment's manual first. If the manual does not back it, it needs an explicit human override and admin approval.

Knowing who signed what

Optional electronic signature on permit approval and close, tied to the exact content that was signed, with a warning if it changed afterwards.

The questions this floor asks, and what comes back.

Examples of shape only. The real content comes from your own manuals and drawings.

What is the recommended flush plan for this pump seal?

Comes back withThe seal manual section with the page. If the manual does not say, the answer says so and logs the gap.

What PPE does the permit need for opening this line?

Comes back withSuggestions grounded in the manual, each marked with where it came from, for a person to review before the permit is created.

The modules that matter most here.

Near-miss register

Anonymous option, routing, photos, and clustering of what keeps recurring.

Permits with gas tests

Category-aware PPE, risk matrix, and a record of who held custody when.

Checklists

Run with progress tracking, flag a step, and attach a photo as evidence.

Signatures and audit

Optional e-signature with re-authentication, and sealed field-level history.
